To apply as a diverse supplier, assemble an application package which contains the items listed below, as well as any other information you feel is relevant. Send your package to: 

Include:

• Name and address of your company
• Statement of the product(s) or service(s) your company provides
• A description of how your company qualifies as a small, minority-owned or woman-owned business, copies of any minority-business enterprise certification you have obtained
• A brief "biography" of your company -- when your company was established, number of employees, any awards won and certifications earned, etc.
• Brochures which describe your company and/or product
• List of current and past customers
• Any other information that will help 3M understand your business
