The 3M™ Littmann® Electronic Stethoscope Model 3000 Medical School Offer ("Offer") is open only to members of a medical school faculty who are authorized agents of Eligible Medical Schools (defined below) located in the 48 contiguous United States and the District of Columbia who use stethoscopes in the course and scope of their teaching duties (inclusive of meeting all Offer-related eligibility and participation terms and conditions, a "Participant"). Offer void where prohibited or to the extent restricted by law.
An Eligible Medical School is: (a) one of the 23 colleges in 26 locations accredited by the American Osteopathic Association (available at http://www.aacom.org); or (b) any post-graduate school that leads to an M.D. degree and which have been accredited by the Liaison Committee on Medical Education (LCME). The LCME is sponsored by the Association of American Medical Colleges and the American Medical Association. The LCME is the nationally recognized accrediting authority for medical education programs leading to an M.D. degree in the United States (list of accredited medical schools available at http://www.lcme.org/start.html).
To participate in the Offer, visit www.littmann.com and click on the link to the official offer website, or go directly to the official offer website at www.littmann.com/medicalschooloffer (“Site”) commencing at 12:00:00 AM Central Time (“CT) on February 1, 2008 through and including the earlier of 11:59:59 PM CT on July 31, 2008 or when supplies of the promotional Stethoscopes (defined below) are exhausted. Truthfully and accurately complete, as directed, all mandatory information requested in the online Offer Order Form and click on the “Place Order” icon, which confirms an Order and additionally confirms your agreement to these Official Offer Terms and Conditions and other terms and conditions posted on the Site.
Upon Order placement, a Stethoscope will be sent to the Eligible Medical School c/o the authorized Participant at the mailing address specified in the Order Form (e.g. which must be the eligible medical school). Please allow approximately 7 business days for receipt from the date of a confirmed order.
By clicking on the “Place Order”, you agree that: (a) you are an authorized agent of the Eligible Medical School and will accept delivery of the refurbished 3M™ Littmann® Electronic Stethoscope Model 3000 (“Stethoscope”) on behalf of the Eligible Medical School; (b) you will use the Stethoscope for teaching/educational purposes; (c) you agree that Sponsor may contact you up to three (3) times via telephone or email for a one-year period following receipt of your Stethoscope for the purpose of obtaining feedback (no more than 10 minutes per contact session) about the Stethoscope in the course and scope of your educational use, teaching experiences and/or clinical use concerning the same; and (d) you agree that, should another authorized agent from your medical school request a 3M Littmann Electronic Stethoscope Model 3000 donation, 3M Littmann may provide your name to that individual as someone who has already received a 3M Littmann Electronic Stethoscope Model 3000 donated to that school.
