WHO IS ELIGIBLE
New certified doctors of practices in good standing using trained and certified laboratories in good standing.
DURATION
One-year‚ which starts at the practice’s certification date.

Lava restorations that are approved indications for the Lava C.O.S.
RESTORATION ELIGIBILITY
Practice
The Lava restoration doesn’t fit the patient and wasn’t seated.
Lab
The Lava restoration fits the unmodified SLA model‚ and the Lava restoration was made using approved processes.
If a Lava Zirconia coping was created using recommended design processes and parameters and does not fit the unmodified SLA model‚ the lab may be eligible to receive credit for a new block under the standard Lava Zirconia Quality Policy.
EXCLUSIONS
Lava restorations rejected for reasons other than fit‚ e.g.‚ shade
Cases that the laboratory or 3M ESPE recommended abandoning
Cases completed showing significant disregard to trained practices‚ at the discretion of 3M ESPE
